CRNA needed in Phoenix, Arizona for urgent dates!

Start Date: As soon as credentialed
Contract Length: 3 months, ongoing
Shift Details: 6:30a-5p. 30 minute unpaid lunch.
Schedule: 4 10hr shifts. 40 hours/week. will consider providers for 3 12's or 4 10's and are in need of July and August coverage. No call. No overtime. Providers must have FT availability and minimal time off requests. MAY consider a provider for consistent PT help IF LOCAL.
Work Details:
· Support Staff:
· Amount of Coverage Needed per Month: 40 hours/week. Full time coverage needed.
· Patients per day: Average of 3-5 outpatient surgery/day. Endo cases can be 10-12/day.
· Patient Population/Age: Adult-Geriatric
· Department Description: Manage complex cases in care team environment. Experience includes but is not limited to: General, Regional, GI, ENT, Ortho, Endo, Podiatry, Urology, Neuro (spines and heads), Robotics, Vascular, Thoracic, Plastics, Trauma. Will work in a care team model, 100% medically directed. No OB/Peds

Requirements:
· Board Certification: Required
· License Required: Active AZ license. Not accepting unlicensed candidates at this time.
· Certifications Required: BLS, ACLS
· Minimum Experience Required: 1-3 years
Credentialing Timeframe: 60-90 days
