Schedule: Mon-Fri Coverage; Site only is open to providers that are not local CONTINGENT that the provider is committed to working TWO CONSECUTIVE WEEKS PER MONTH.

Practice Setting:
• Hospital
Requirements:
· Board Certification: Eligible
· License required: AZ LICENSE IN HAND
· Certifications required: ACLS, BLS, PALS
· Minimum experience required: Competent with nerve blocks
Credentialing Timeframe: ASAP, will be fast, need clean file to complete
