• Facility: Acute Care Hospital
• Patient population/age: Mainly Adults and geriatric
• Case mix : General, ambulatory, gynecology, ENT, ortho, neuro. Out of OR locations: endoscopy, electrophysiology, interventional radiology. No thoracics, cardiac, peds, or OB.
• EMR: EPIC

Travel and lodging accommodations:
• Flight, hotel, rental car or mileage.
