End Date: Ongoing
Coverage Type: Scheduled Clinical Hours Only
Schedule: Monday- Friday clinic only (8am-5pm). 3 weeks a month is needed

· Support staff: Yes
· Patients per shift: 12-15
· Providers Requested: 1
· Trauma Level: 3
· EMR: Epic
· Skills required: outpatient
Practice Setting:
• Outpatient
Requirements:
· Board Certification: Yes
· License required: Active OR License
· Certifications required: ACLS, BLS
Credentialing Timeframe: No privileges required so they can start as soon as set up

• 13 week with possibility of extending
• No on call
