About the clinician's workday
The Licensed Clinical Social Worker will provide comprehensive community-based psychiatric support services within an ACT team environment, maintaining a caseload of up to 10 patients requiring intensive therapeutic intervention. The clinician will deliver individual therapy sessions, crisis intervention services, and psychosocial rehabilitation in home and community settings while conducting thorough biopsychosocial assessments and developing individualized treatment plans. Primary responsibilities include coordinating care with psychiatrists, nurses, and multidisciplinary team members to monitor behavioral health symptoms and support medication adherence. The position requires maintaining detailed documentation of all therapeutic interactions and active participation in team meetings and case consultations within the ACT framework.
