Required clinical activity in the last 24 months (case logs): Applicant must provide documentation (case list) of provision of clinical services (100 patients) representative of the scope and complexity of privileges requested during the previous 24 months. Case list must include diagnosis or procedure and date, and facility where performed.

Will provider work solo? If not, other providers specialties working with them: No, they will work with other physicians and NPS

Licensed Candidate: Yes, Washington license is required since we will be requesting urgent credentialing. IMLC will not suffice for us to start credentialing – Washington license must be in hand.

Required certifications (Must be from American Heart Association or American Red Cross as applicable): BLS
