Home Health NP/PA Needed in Illinois
Target Start Date: As soon as credentialed
Contract Length: Short-term or long-term options available (Full-time, Part-time, Ongoing Contract)
Schedule: Flexible, Monday–Sunday, 7a–7p (No nights or weekends required)
Call Details: No call

Job Details:
Conduct pre-scheduled in-home health evaluations for Medicare Advantage, Medicaid, and select commercial plan members
Visits include medication and medical history review and physical evaluation (no prescribing, ordering labs, or altering treatment plans)
Average visits per day: 6–9
Travel support available for qualifying providers (airfare, hotel, rental car, gas stipend)

Requirements:
Board Certification: Required
Active Illinois License
Education: Master’s degree required
Credentialing Timeframe: ~30 days

Location: Counties available all over Illinois; please inquire
