Job description

family Nurse Practitioner locum opportunity available in Bowling Green, KY with a hybrid telehealth and home health model.

Schedule:
• Monday–Friday, 8a–5p (1-hour lunch)
• Full-time (40 hours/week)
• No call
• Start: April 2026 (ASAP available)
• Month-to-month assignment with option to extend

Practice:
• Hybrid model: ~70% telehealth, 30% home visits
• 12–14 telehealth visits/day or 6–7 home visits/day
• Assessment-only role (no prescribing)
• Pediatric and adult patient population
• Focus on annual risk assessments and chronic condition reviews
• Escalate care needs to PCP
• Equipment provided

Requirements:
• Board Certified Family NP
• Active KY license (licensing not available)
• Medicare credentialing required
• Comfortable with telehealth, home health, and pediatric patients

Additional Info:
• Paid orientation and training
• Mileage reimbursed for home visits
