Responsibilities include clinical oversight, H&Ps, chronic disease management, interdisciplinary team coordination, and participation in on-call rotation.
Requirements: Active MA license, BC IM/FM (Geriatrics preferred), DEA/CDS, supervisory experience, and 1+ year working with frail/elderly populations.
Required documents: Updated CV, MA license copy, DEA/CDS, board certification proof, immunization records, and malpractice history.
