Location: Lebanon Indiana

Start Date: ASAP
Contract Length: At least 3 months, likely longer

Schedule: 4 days in office, 8 call days/ month (negotiable) - It is 24hr call, 8 days/ month on average, willing to work with the schedule.

Clinic details
Practice setting - Acute Care Hospital
Patients per shift: 12-15 patients per day
EMR- Meditech
Cases/Procedures - common patient diagnosis are MS, fibromyalgia, migraines of varying kinds, dementia, and Alzheimer’s

Requirements - Board Certification, BLS, Indiana License, DEA
