Requirements-
Active, unrestricted Idaho medical license (in hand) or IMLC accepted
DEA
Board Certified
BLS
Proof of COVID vaccination
NPDB report (issued within the last 30 days)
CV in MM/YYYY format with all 30+ day gaps explained
