Schedule: We are looking for two providers for this need. Ideally, we would like to privilege them at QVH & PVH for
locum needs as they arise there as well. Specific Dates Needed (Provider must have availability or will not be considered):

· Patients per shift: 10-15
· Providers Requested: 2
· Trauma Level: 1
· EMR: Epic
Practice Setting:
• Inpatient
Requirements:
· Board Certification: Yes
· License required: Active CA License
· Certifications required: ACLS, BLS
Credentialing Timeframe: 30-60 Days

• 13 weeks with option to extend
• No on call
