• Support staff: MA, RN, APP will be solo
• Coverage Type: All patients
• Schedule Details: 8a-8p

• Procedures: all required urgent care testing, diagnosis, suturing and spliting
Requirements:
• Board Certifications Req’d/Preferred: Required
• State License(s): Oregon
• Certifications: ACLS
• CSR Req’d: NO
• DEA Req’d?: Yes
• Years of Experience Req’d: 3 years
Credentialing Timeframe: 4 weeks

• 13 weeks with option to extend
