Patient Population: Adults only
Setting: Clinic and hospital-based urology
Responsibilities: Provide clinic coverage and night call coverage; general urology procedures required; 2-5 patients per day
Required Procedures: General urology procedures
EMR System: EPIC
Support Staff: Not specified
Days Per Month: Varies based on coverage schedule
On-Call/Weekends: Occasional weekends; night call 5p-7a; 24-hour call option if provider prefers no clinic

Submission Requirements:
BC or BE within 5 years of residency
Indiana license at time of submission or IMLC
CDS Certification and DEA Registration
ACLS Certification
Robotics training highly preferred
List availability at time of submission
